You may request to inspect or receive a copy of your health records.

You may request limits on how we use or disclose your information. We are not required to agree to all requests.

d. Right to Confidential Communications

You may request that we contact you in a specific way (e.g., only by email or at a specific address).

e. Right to an Accounting of Disclosures

You may request a list of certain disclosures of your PHI.

f. Right to a Paper Copy

You have the right to receive a paper copy of this Notice at any time.

7. Our Responsibilities

We are required to:

  • Maintain the privacy and security of your PHI

  • Provide you with this Notice of our legal duties and privacy practices

  • Notify you in the event of a breach of your unsecured PHI

  • Follow the terms of this Notice currently in effect

8. Telehealth & Electronic Communication

If you participate in telehealth services:

  • We use secure platforms to protect your information

  • You are responsible for ensuring your environment is private

  • Electronic communication carries some inherent risks despite safeguards

9. Changes to This Notice

We reserve the right to change this Notice at any time. Updated versions will be posted on our website with a new effective date.

10. Complaints

If you believe your privacy rights have been violated, you may file a complaint with:
